Luis I. Salazar, M.D. founded Provida Family Medicine in 1992. Dr. Salazar received his medical degree from the University of Illinois at Chicago. He completed his residency in Family Practice at St. Joseph Hospital in Chicago, where he obtained training in internal medicine, psychiatry and psychology, pediatrics and community and adolescent medicine, obstetrics, and gynecology, Dr. Salazar has practiced immediate care medicine as well. He has been on staff at the Lake County Health Department. He is a diplomate of the American Board of Family Practice. He is past delegate to the Illinois State Medical Society and the Lake County Medical Society. Dr. Salazar is on staff at Advocate Condell Medical Center, Northwestern Lake Forest Hospital, and Vista Medical Center. In his free time, Dr. Salazar enjoys reading a wide array of historical and political subjects. He is fond of eating red hots, lemon heads, swimming, biking and listening to improvisational jazz.
Peter Nani, M.D., started at Provida Family Medicine in 2021. Dr. Nani received his medical degree at the American University of the Caribbean School of Medicine and completed his residency in family medicine at Saints Mary and Elizabeth Medical Center in Chicago. Prior to his medical career, Dr. Nani completed a Ph.D. in classical languages and literature (ancient Greek and Latin) and served as a professor at both Georgetown University and the University of Iowa. Dr. Nani stresses the importance of building collaborative relationships with his patients to maximize the patient’s healthcare needs. In his spare time, he enjoys cooking, working in the yard, maintaining his tropical fish tanks, and listening to his extensive jazz collection. He is also a devout Notre Dame fan.
Rosalva Acosta, P.A.-C.,began working at Provida Family Medicine in 2002, Rosalva earned her physician assistant certificate at Western University of Health Sciences in Pomona, California. She completed her bachelor’s degree in biology at California State University, Fresno. Rosalva believes in building strong patient-provider relationships in order to help patients maintain their optimal health and reach treatment goals. Rosalva strives to deliver high-quality health care by collaborating with patients, their families and caregivers. She is dedicated to providing continuing and comprehensive health care for the individual and entire family. In her spare time Rosalva enjoys spending time with her husband and 3 children as well as traveling to visit family on the west coast.
Caitlin Chandran MSN. APRN-FPA. FNP-BC has been with Provida Family Medicine since 2014. Caitlin earned her Advanced Practice Nurse degree from the University of Illinois at Chicago. She is a board certified Family Nurse Practitioner. Prior, she received her Bachelor of Science in Nursing from Illinois Wesleyan University. Caitlin received the majority of her training in a Family Practice setting. Her background in nursing has allowed her to take what she considers a special approach to providing patient care: holistic care, emphasis on listening, involving the patient in their medical care and decision making, and overall focus on preventive medicine. Caitlin had 5 years of experience as an RN prior to her current practice as a nurse practitioner. In her spare time she enjoys spending time with her husband, two children, and extended family. She loves cooking, gardening, watching movies, Christmas, and football season- “Go Denver Broncos!”
Megan Kahue P.A.-C started with Provida in 2019. Megan graduated from Dominican University with prior experience in geriatrics and memory care including hands-on patient care and working as a Lead CNA. Megan’s interests include internal & adult medicine, primary care, under-served healthcare, patient education, HIV/AIDS medicine, and infectious diseases. Megan strongly believes in the use of evidence-based medicine to guide her clinical decisions. She also seeks to establish good relationships with her patients, including providing patient education to help empower them to make health lifestyle choices. In her free time Megan likes to be outside in any capacity, but she mostly enjoys gardening, biking, camping, and kayaking. Her love for the outdoors comes from her frequent travel to South Carolina, where she grew up, and visiting Hawaii with her husband’s family in the winters to enjoy the sunshine and good food.
Ksenia Samvel, P.A.-C, has enjoyed working at Provida Family Medicine since 2014. She completed her Masters of Physician Assistant Studies from the University of Texas Pan American. Upon completing high school as a junior, Ksenia transitioned into UTPA where she earned the distinguished honor of graduating with degree in Bachelor of Science Biology, Cum Laude and a Minor in Chemistry. During Physician Assistant program Ksenia completed extensive training in Internal Medicine. Since graduating, Ksenia has worked in Internal Medicine, Nephrology, Emergency Medicine and Family Practice. She speaks Russian, English and Spanish languages. Outside of work, Ksenia enjoys time with her family, playing with her three dogs, and touring the Midwest.
Susie Gonzalez, APN, FNP-BC joined Provida Family Medicine in 2020. She received her Advanced Practice Nurse degree from North Park University. She is a board certified Family Nurse Practitioner. She received her Associate Degree in Nursing from the College of DuPage and her Bachelor of Science in Nursing from the University of Illinois at Chicago. Susie has eight years of experience as an emergency room nurse. She is interested in patient empowerment and health literacy. Susie believes that patients should be provided health information in terms that they can understand. She applies evidence-based practice to make patient care decisions. In her spare time, Susie enjoys spending time with her daughter, two dogs, and extended family. She likes to be active and can be found at CrossFit or running. Other hobbies include watching movies, reading, and eating good food. She is fluent in Spanish.
Dominic Alesandrini, P.A.-C began working with Provida Family Medicine in 2019. He received his Master’s degree from Marietta College of Marietta, OH with an emphasis in Family Medicine. Dominic is interested in utilizing evidence-based medicine, patient and family education, and patient-centered care to focus on the needs of patients and their loved ones. Prior to working as a PA, Dominic served as a Wilderness First Responder in the Sangre de Cristo Mountains backpacking and in the subzero winters of the Minnesota Boundary Waters teaching kids about winter camping where he gained a healthy appreciation for providing healthcare and service to others. After double majoring in Spanish and International Studies at Bradley University, he was formerly a trail guide, a logger, a rock climber, a sailing instructor, an interpreter, managed a dude ranch, and served as an EMT. He enjoys snowboarding, volunteering, cooking, playing the harmonica, and trying new things. He also makes a mean lasagna.
