Luis I. Salazar, M.D. founded Provida Family Medicine in 1992. Dr. Salazar received his medical degree from the University of Illinois at Chicago. He completed his residency in Family Practice at St. Joseph Hospital in Chicago, where he obtained training in internal medicine, psychiatry and psychology, pediatrics and community and adolescent medicine, obstetrics, and gynecology, Dr. Salazar has practiced immediate care medicine as well. He has been on staff at the Lake County Health Department. He is a diplomate of the American Board of Family Practice. He is past delegate to the Illinois State Medical Society and the Lake County Medical Society. Dr. Salazar is on staff at Advocate Condell Medical Center, Northwestern Lake Forest Hospital, and Vista Medical Center. In his free time, Dr. Salazar enjoys reading a wide array of historical and political subjects. He is fond of eating red hots, lemonheads, swimming, biking and listening to improvisational jazz.
Rosalva Acosta, P.A.-C.,began working at Provida Family Medicine in 2002, Rosalva earned her physician assistant certificate at Western University of Health Sciences in Pomona, California. She completed her bachelor’s degree in biology at California State University, Fresno. Rosalva believes in building strong patient-provider relationships in order to help patients maintain their optimal health and reach treatment goals. Rosalva strives to deliver high-quality health care by collaborating with patients, their families and caregivers. She is dedicated to providing continuing and comprehensive health care for the individual and entire family. In her spare time Rosalva enjoys spending time with her husband and 3 children as well as traveling to visit family on the west coast.
Caitlin Chandran APN, FNP-BC has been with Provida Family Medicine since 2014. Caitlin earned her Advanced Practice Nurse degree from the University of Illinois at Chicago. She is a board certified Family Nurse Practitioner. Prior, she received her Bachelor of Science in Nursing from Illinois Wesleyan University. Caitlin received the majority of her training in a Family Practice setting. Her background in nursing has allowed her to take what she considers a special approach to providing patient care: holistic care, emphasis on listening, involving the patient in their medical care and decision making, and overall focus on preventive medicine. Caitlin had 5 years of experience as an RN prior to her current practice as a nurse practitioner. In her spare time she enjoys spending time with her husband, two children, and extended family. She loves cooking, gardening, watching movies, Christmas, and football season- “Go Denver Broncos!”
Emily Gebhardt PA-C has left the practice to take a position closer to home in Milwaukee Wisconsin. Emily Gebhardt worked at Provida Family Medicine since 2014. She is sad to leave behind so many wonderful patients and will miss them. Many of the patients she served made positive impacts on her life and she will cherish her time at Provida Family Medical Center.
Emily Gebhardt PA-C has been working at Provida Family Medicine since 2014. She especially enjoys seeing children and interacting with families, whether they are coming in because of an illness or for a routine physical. Emily earned her Master’s degree in Physician Assistant Studies at Carroll University in Waukesha, Wisconsin. She was trained in several areas of primary care such as family medicine, pediatrics, and women’s health, in addition to specialty areas including psychiatry and surgery. Her training included an international medicine rotation in Yantalo, Peru as well. She attended the University of Wisconsin-Madison for her undergraduate education, earning her Bachelor’s degree in biology. Along with college courses, Emily learned Spanish when she lived in Spain for a semester taking classes in Spanish and living with a host family. In her free time, Emily likes to travel, dance, cheer for the Packers and Badgers, and spend time with her friends and family.
Ksenia Samvel, P.A.-C., has enjoyed working at Provida Family Medicine since 2014. She completed her Masters of Physician Assistant Studies from the University of Texas Pan American. Upon completing high school as a junior, Ksenia transitioned into UTPA where she earned the distinguished honor of graduating with degree in Bachelor of Science Biology, Cum Laude and a Minor in Chemistry. During Physician Assistant program Ksenia completed extensive training in Internal Medicine. Since graduating, Ksenia has worked in Internal Medicine, Nephrology, Emergency Medicine and Family Practice. She speaks Russian, English and Spanish languages. Outside of work, Ksenia enjoys time with her family, playing with her three dogs, and touring the Midwest.
Michael Mesirow, PA-C joined Provida Family Medicine in 2018. He completed his Master’s Degree in Physician Assistant Studies at Rush University in Chicago, IL. An English and Spanish speaker, Michael is passionate about family medicine and has received extensive training in primary care. Michael is deeply committed to using evidence-based medicine to guide his clinical approach. Michael is qualified to treat a broad spectrum of conditions among patients of all ages. He is especially interested in musculoskeletal conditions, skin problems and diabetes management. Michael has a strong belief in partnering with his patients to develop medical goals that lead to better health. He feels patient education is central to the role of a medical provider. In his spare time, Michael enjoys spending time with his wife and two children.
