Emergencies

Always call 911 in life-threatening emergencies.
As soon as you or a family member is able,
please call our office so that you may follow up with us after the hospital or emergency department visit.

After Hours Medical Care

If your medical condition is not life-threatening,
you may call 847-548-2200. The after hours voicemail message will contain the phone
number of the on-call provider that will
assist in taking care of your medical needs.

Appointments

Book your appointments 24/7 through the
Healow App. You may also call 847-548-2200 during office hours to make an appointment.
We offer same day, early morning, late evening and Saturday appointments.

Please bring to every appointment:

• Photo identification
• Insurance Card
• Co-payment or co-insurance
• Bottles of all medications you are taking
• Medical immunization records
• Information from other medical facilities
• Your health questions and concerns

Please arrive 15 minutes prior to
each appointment for registration

Appointment Confirmations

Provida Family Medicine uses an automated
confirmation system. When we call to confirm a
future appointment, please listen to the entire
message, then PRESS 1 to confirm, PRESS 2 to cancel, or PRESS 3 to change your appointment
to another date and time.

Please notify us of APPOINTMENT CHANGES by NOON the day before
the appointment.

Minors

All patients younger than 18 years of age must be
accompanied to their appointments by a parent
or legal guardian. Written authorization by a parent or guardian must be provided for another individual to accompany the patient.
A consent form is also available at registration.

Payment

Payment for your visit is made during the check-in process prior to your appointment. We accept cash, check, debit and several major credit cards. We accept most major insurance plans including HMOs and Medicare. We also have special pricing for our patients that do not have insurance.

We provide equal access to all patients
regardless of insurance status.

Healow & Patient Portal

With Healow, gain easy, private access to your medical information online whenever and wherever you have internet access. All we need
is your email address to get you started,
or download the Healow App and enter code BIGDAA to sign up.

portal frequently!

Refills

We use an automated prescription refill service. Please call your pharmacy directly with your prescription number as soon as you notice you
are running low on your medication. The pharmacy will contact us to process your refill, allowing for better accuracy and service.
Our staff handles refill requests Monday
through Friday during office hours.

Please allow 48 hours for all
prescription refills.

Telephone Calls

We have staff available during office hours to answer all calls. If your call is routed to our voicemail system because we are assisting other patients, please leave us a message. We check
our voicemail throughout the day, and one of
our staff members will return your call promptly.
